All defense mechanisms are effective in managing uncomfortable inner conflicts, feelings and thoughts. We use them when dealing with important relationships and challenging situations.
Level I: Psychotic, pathological defenses involve a pronounced break with objective reality as in psychosis. Although this is not common among healthy adults in day time, we use it in dreams. They are common during childhood. They can also be used by adults when under extreme pressure. Typical situations of this is when being exposed to a traumatic event.
Examples of this is being convinced that the other person is evil, also called delusional projection which includes attributing threatening thoughts, emotions and impulses that are not based on reality to others as in paranoia. It also includes blaming others, society, history, economy for own failures.
Another example is psychotic denial. It is the refusal to accept the reality or facts, acting as if a threatening or painful event, thought or feeling did not exist or something the person do not want to admit.
Finally, psychotic distortion involves fundamentally reshaping external reality as when thinking one is in another place.
